W-2 and Tax Information for 2016
(Updated February 17, 2017)
2016 W-2’s have been mailed out and may take up to 10 business days to receive. If you have not received your 2016 W-2 by Friday February 17th, you may contact our accounting department at (567) 661-0609 or w2info@job1usa.com. Please leave a detailed message and your call will be returned within 2 business days.
To request a W-2 reprint, you will need to mail us the following:
- Your name
- The last 4 digits of your Social Security number
- Your phone number
- Your current mailing address
- The 2016 W-2 reprint has a Processing fee due in the form of a check or money order, made payable to Job1USA in the amount of $5.00

If the request is received after 3/31/2017, the processing fee will be $10.00.
For W-2 reprints for tax years 2015 and prior the processing fee will be $15.00.
Address Changes: All address changes for 2016 W-2’s have been processed. Please contact the accounting department if you had an address change and not received your W-2 as a W-2 reprint will need to be requested.
Employees Only: If you have had a change of address within the past year, please verify that your current address is in our system for us to mail your 2016 W-2 tax information. You can do this by logging into the pay history web page and checking the address listed there. If you need to update the information, please fill out this form to send the updated information to us.
This should be done before January 15, 2017. W-2 forms will be mailed out no later than January 31, 2017.
